Registry Easy is my personal choice for a registry cleaner.
Registry Easy is easy to use and also performs has a registry backup that you can use as a fail-safe measure, in case you want to roll-back any changes.
The interface is easy to use, and you won't feel confused trying to find your way around the program.
I've had a Windows system speed up significantly after running Registry Easy. This particular system is used for testing lots of software and every few months the performance slows to a crawl, and the entire system is just reformatted and Windows reinstalled from scratch.
Instead of doing a format and reinstall, this time round I ran Registry Easy on it. It found many errors and fixed them. The system now feels very spiffy and there're no more error popups.

Perfect Optimizer comes in a close second. It's pretty fast and manages to live up to its name, "Optimizer".
It scanned through the test machine's registry fast and found quite a few problems in it. It's also trivial to fix these problems using this tool. After Perfect Optimizer is done, the computer felt faster and the startup problem which it had prior to running Perfect Optimizer also disappeared.

RegTool comes in third, and is easy to install. The user interface is also very simple to use, and there is nothing confusing about it. However, one thing is that it detect more registry errors than other registry cleaner software. Not sure whether this is a good thing.

Disadvantage
- Not sure if the scanner is being overzealous; may or may not be a good thing.
- Not as comprehensive as I'd like it to be.
RegTool functions well as a registry cleaner if that's what you're looking for, but if you're also interested in an all-round system optimization tool, the other registry cleaners reviewed seemed more comprehensive.
